Your first visit at CovenantCare in four steps. Step 1: Schedule. Book online or call any of our three Tennessee clinics. Most first appointments are available within a week. Step 2: Complete intake. Receive a secure digital intake link after booking. Fill it out from home — no paper forms in the office. Step 3: Your visit. Plan for 30 to 45 minutes. Your provider reviews history, performs an exam, and lays out a plan. Step 4: Set up the portal. Before you leave, we activate secure messaging, lab results, refills, and bill pay — all online.

What to bring

A checklist so you arrive ready.

A current photo ID (driver's license or similar)
Your insurance card
A list of all medications you take, including dosages and how often
Any recent medical records, lab results, or imaging — if you have them
A list of questions or concerns you would like to discuss
