Four steps, from booking to your first appointment.

Step 1
Schedule
Book online or call any of our three Tennessee clinics. Most first appointments are available within a week.

Step 2
Fill out your intake forms
After you book, we send you a secure link. Fill it out at home. There are no paper forms at the office.

Step 3
Meet your provider
Your provider goes over your health history, does an exam, and makes a plan with you.

Step 4
Set up the portal
Before you leave, we turn on your patient portal for secure messages, lab results, refills, and bill pay.
What to bring
A checklist so you arrive ready.
- A current photo ID (driver's license or similar)
- Your insurance card
- A list of all medications you take, including dosages and how often
- Any recent medical records, lab results, or imaging — if you have them
- A list of questions or concerns you would like to discuss
